Siouxland parents who need assistance providing Christmas gifts for their children should fill out this application and register with Goodfellow Charities, Inc.

All application cards will be mailed using the address supplied. Cards will be mailed as early as possible.

The first batch of cards should be sent by Friday, November 15.
